Ah ok.
Just cust & pasrz the form to your page (between the tags <form> and </form> in _contact.htm) and upload contact.php to your web server.

Hello,
Make a "thank you" page, for example thankyou.htm and in the contact.php page change :
header("Location: ./index.htm?page=_contact_message.htm");
by
header("Location: ./thankyou.htm");

Thanks for your help, all right now, lit only problem with board's :

1 - Message Borderers is small i will be alitel high like orginal border.
2- Other Borderers are not series with each you se in the pictuer beter.

what I must doing for fix it?

Hi,
The problem is coming from your css. I suggest you too look at flash template 9 css.
But the easiest way to make forms like this is to use tables tags (<table>,<tr>,<td> etc...)
