Hoping someone can help me with header

Hi I just discovered this website earlier today and I downloaded the flash header (banner 201)

I would like to make my header (banner) at the top of this page:
http://standoutwebdesigns.com/attic-insulation.html
flash across just like the one I downloaded.

I tried to follow the instructions on how to customize it like replacing the red values by mine but it's not working. Even if I copy and paste the original code it's not working.

Here's an example:

(I uploaded the files to my server that came in the zip folder)

Can anyone supply me with the code/HTML so I can just copy and paste it?

Help will be greatly appreciated.

When you download the banner 201, you will have an index.htm in the zip archive. This file show you how to use the banner in your own html pages.
