Default Contact Form in free template

How do I set up the "Contact Form" so that someone can send their information to me? I have tried out the form and I keep getting the 401 error. My site is www.memoriesbyjason.com (template 9) and www.storehousemarketing.com (template 7)

Thank you for your help!!

Default Contact Form

The form has been uploaded to my webhost and I do have php capabilities. I actually have a myPHPadmin as well as the ability to build a mySQL database from my web host (www.justhost.com).

Thank you:
Jason

Default send mail form php from a template

Can you try to make a php page with that code to see if you are allowed to send mails from php :
<?php phpinfo(); ?>

I think you should contact your hosting company about that problem.
